Nursing Case Management
We have RNs and LPNs on staff for initial client assessments and ongoing evaluations. This service includes development of a personal care plan based on our clients’ specific needs at no extra charge.
Medication Reminders
Reminders, assistance with opening medication containers and reading prescription labels. Our direct care staff cannot administer medication; however, they can remind clients to take them, and assist with opening medication containers and reading prescription labels as needed.
Hospice
Our staff is skillfully trained to provide end-of-life care that helps clients and their families. Community Caregivers works in conjunction with area hospice providers to assist their clients with hospice care.
